Patient/Client Information
Referrals: Physician referral is not required. Clients should contact Dr. Zukiwski personally to book appointment. Letter of referral from physician or other health care professional is welcome but not required. An appointment can usually be scheduled within 1-2 weeks.
Information Session: A free information session is available for those interested in brain mapping and brain biofeedback (“neurofeedback”).
Fees: Assessment or treatment sessions are $160 per one-hour session. Quantitative EEG “brain mapping” is typically $900 total. This includes a 3-hour appointment to collect the EEG data, an expert report from the US, time for data and report preparation, and a one-hour session to explain the results. *Prices may be subject to change without notice.
Payment: Cash or cheque. No debit or credit.
Missed Appointments: Dr. Zukiwski reserves the option to charge whole or partial session fee for appointments missed without 24 hour notice.
Insurance: Most third party insurance plans have coverage for services provided by a psychologist ranging between $500 to $1200 per year. Alberta Health Insurance does not cover the assessment or treatment.
Veterans and Military: Dr. Zukiwski is an approved provider for veterans and active service members of the Canadian Forces.
Number of sessions: The goal is permanent change without medication. The change is usually control or elimination of symptoms and improvement in functioning. Depression, anxiety, migraines, and insomnia typically respond rapidly and may improve with as little as 6-10 sessions. For most conditions a minimum of 10 to 20 sessions of brain biofeedback are required. Some people may require more sessions.
Frequency of sessions: Treatment is typically once or twice per week.
Ages: Children, teens, adults. Minimum age is 4 years.
Medications: This treatment does not involve the use of medications. In some cases, medication may be withheld prior to the QEEG assessment (with physican approval). However, typically clients can continue taking medications already prescribed by a physician.
